Allow for multiple forms design under one LogBook category

We have a diverse workforce where a 'onboarding' checklist would differ depending on the employee group you belong to i.e. we have engineers as well as office staff.

Allowing for different template designs to be selected depending on the employee type would save us having to create similar LogBook types for the same thing
