Is it possible to have an audit field in logbook to identify when a record changed/deleted?

We know that the training logbook had an out of date "training event" for an employee and needed to be renewed - it was there when we ran a report. A few weeks letter when we ran the same report the out of date "training event" had been deleted but the training event hadn't happened.
